Which forms are available
Five ACORD forms are supported:
- ACORD 80 — homeowner application
- ACORD 88 — personal auto application
- ACORD 125 — commercial insurance application
- ACORD 126 — commercial general liability section
- ACORD 140 — property section
State-specific editions
Several states mandate their own edition of a form. Quotamation selects the right one automatically from the risk address on the quote, so there is nothing to choose — ACORD 80, 88, and 125 all ship with state editions, and everything else falls back to the national edition. On the ACORD 125, state editions also differ in how many prior loss rows they allow, and the generated PDF respects whichever limit the chosen edition sets.
Quote first, then download
Forms are built from quote data, so you need to run the quote before they appear. Until a quote has been attempted on the visit, no forms are offered for download. Run the quote, then come back.
Where to download them
- Open the quote you want forms for
- Find the ACORD Forms section and expand it
- Click the download button next to the form you need
Each form downloads as a filled PDF you can review, finish by hand where needed, and send on to the carrier.
If you don't see an ACORD Forms section at all, ACORD form generation isn't switched on for your agency — contact your account manager to have it enabled.
